China Scientific Instruments Hubei Co., Ltd. was commissioned by the Wuhan Donghu New Technology Development Zone Public Health Service Center to conduct a price inquiry for its required third-party epidemic prevention and control service project for vehicles entering and leaving Wuhan from the Donghu High-tech Zone expressway toll station.

III. Supplier Qualification Requirements:

1. Suppliers must be independent legal persons, organizations, or natural persons registered (three certificates in one) in accordance with the law within the People's Republic of China;

2. Suppliers need to provide a letter of declaration stating that they possess the necessary equipment and professional technical capabilities to perform the contract;

3. Suppliers should possess a "Pest Control Service Agency Qualification Certificate" and the service scope should include disinfection services of level B or above and pest control agency AA-level qualifications;

4. The project manager assigned by the bidder should possess a "Pest Control Worker" technician secondary or above vocational qualification certificate, and must provide the project manager's social security proof and copy of the employment contract from the bidding unit for the past three months;

5. Suppliers shall provide a written statement that in the three years prior to participating in this bidding activity (if less than three years, since the company's establishment), they have no major illegal record in their business activities, and all the materials and documents provided for this project are true, legal, and valid;

6. In the three years prior to participating in this procurement activity (if less than three years, since the company's establishment), the supplier has not been included in the list of dishonest persons subject to execution, abnormal business register of enterprises, major tax violation cases parties, and the list of serious illegal and dishonest behaviors in government procurement on the "Credit China" website (www.creditchina.gov.cn) and the "China Government Procurement" website (www.ccgp.gov.cn), as determined by the query results after the announcement is released;

7. Suppliers need to provide a declaration letter stating that there are no dishonest records in any aspects such as tax payment and social security;

8. Joint bids are not accepted for this project.

Suppliers should submit the sealed response documents and preferential services (the sealed part should be stamped with the supplier's unit seal) before the deadline. Response documents received after the deadline will not be accepted. Once the response documents are submitted, they cannot be changed.
